SPRING2012 REGISTRATION
Register by mail at address below or at the Prouty building on the following dates:
Saturday, January 28th from 9-noon:
Wednesday, February 1st from 5:30pm-7:30pm: Saturday, February 4th from 9-noon:
Wednesday,February 8th from 5:30 pm-7:30pm: Saturday, February 11th from 9-noon:
Fees:
K-8 is $45.00 per player (for the first two players in a family and $35.00 for each additional player)
Pre-K is $35.00 per player. Player must be 4 years old by January 31, 2012.
Important Information:
There will be a LATE FEE of $10.00 for registrations that are post marked after February 13th.

Important Dates:
Picture Day: March 31, 2012 --PICTURES WILL BE HELD AT CHRIST COMMUNITY CHURCH (RAIN OR SHINE) The church is located next to Zearing Park.
Field Preparation Day: Saturday March 31st starting at 8:00 a.m.
Fall Season will run from: April 9th thru May 19th.

I/We hereby agree that Princeton Youth Soccer League (PYSL), its members, coaches, officers, or those members, coaches, or officers of any other organization to which PYSL is affiliated, shall not be liable for any injury or loss which my child or children may sustain while participating in activities of any kind, whether sponsored by, or under the supervision of PYSL or any other organization to which PYSL has an affiliation. We further agree to indemnify and to hold harmless, PYSL, or any affiliated organization, its members, coaches, officers, or designates of any kind, from any claim what so ever. I/We agree we are responsible for the return of borrowed equipment, including apparel, upon demand of PYSL.

Players are NOT allowed to wear jewelry of any kind, to practices or games (including newly pierced earrings). No jeans, jean shorts, tear-a-ways, carpenter pants or any other pants or
shorts with pockets or belts of any kind. Also there will be NO CASTS or gum allowed.
Athletes must provide shin guards and they must be worn to ALL practices and games.
Emergency medical release waivers MUST be filled out PRIOR to any participation.
(This includes practices and games)
PYSL supplies the complete uniform and is required to be worn for
your child to play.
There will be NO REQUESTS for teams or coaches honored other than for a
sponsor or coach's child.
There will be a 50% fee assessed for players that drop out after registration.
NO REFUNDS after teams are picked.
Final Registration deadline is February 17th.
There will be a $25.00 service charge for ALL returned checks.
PLEASE MAKE SURE THAT ALL INFORMATION ON THE FRONT IS FILLED OUT AND THAT IT IS CORRECT. THIS WILL MAKE THINGS RUN MORE EFFICIENTLY.
